Mid '70s Advertising Flexi Record inside Radio Times

There's an interesting topic about advertising flexi disc records on 45.cat at the moment.
I was reminded of a flexi "Paper" record within a copy of "Radio Times". It was a full page advert with the record within which had to be cut out of the page before playing.
I remember it was a male narrator advertising a washing machine(?). To win a prize one had to guess who the narrator was.
The card must've been extremely thin as after a couple of plays on a typical mono record player it cut through into the paper in parts.
Anyone else remember this and can perhaps provide further details or even a scan, say a collector of old editions of RT? Think it was 1974 or 1975.
